Clinic Intake Form
Fill out Dr. Winkler’s intake form. It is 7 pages and comprehensive. And on completion, Dr. Winkler will call you to discuss whether a full assessment for ADHD makes sense for you. This is free.” Action: link to the Yosi online intake form. I can send you the link if you dont still have it.
Intake Form Support
If the idea of filling out a 7 page intake form makes you want to jump out of your skin, you might have ADHD. For a small fee, a trained medical assistant can walk through the intake form with you and take down your answers. After that Dr. Winkler will call you to discuss whether a full assessment makes sense for you
Diagnostic Assesment amd Medication
Dive in, get moving, get answers, and start medication if that’s medically appropriate. Pay for the first appointment and schedule a time to see Dr. Winkler for an ADHD assessment. Full assessment usually takes two appointments over two weeks. Starting medication and getting to a stable long-term dosage usually takes 4-5 appointments over 2-5 months, including the first two.
Depth Psychotherapy
If you’re ready to face your deepest struggles head on, if you’ve tried therapy before but have deeper problems that still effect your life, set up a time to chat briefly with Dr. Winkler to see if this could be a good fit.
